Protected Health Information (PHI) includes any information about your health status, the provision of healthcare, or payment for healthcare that can be linked to you. This includes your eyeglass prescription, vision test results, and related medical history.
How We Use and Disclose PHI
- Treatment: We use your prescription information to fill your eyewear order and coordinate with your eye care provider.
- Payment: We may use PHI for billing and payment purposes.
- Healthcare Operations: We may use PHI for quality improvement and compliance activities.
- Legal Requirements: We may disclose PHI when required by law, such as reporting to public health authorities.

You may also file a complaint with the U.S. Department of Health and Human Services Office for Civil Rights at www.hhs.gov/ocr/privacy. We will not retaliate against you for filing a complaint.
